New to 2009-10, we’ll be accepting registrations below. You will be contacted by Dance-n-Fitness to confirm receipt of your registration.
My submission of the above form indicates that I have read and understand the tuition and studio policies of nine (9) equal payments, Sept.-May. I also agree to pay monthly tuition in full by the first class each month or be subject to a $20 per month late fee. I give Dance-n-Fitness permission to use pictures taken during classes, picture day, and performances for publicity purposes. Dance-n-Fitness, its owner, instructors, and staff will not be held accountable for any injury or loss of property while the above student is attending, coming to, or leaving the school.
You may be asked to sign a similar agreement at your first class. You must bring payment, including registration fee of $35 if registered after August 18th.
